Essential Duties
This position includes, but is not limited to, the following essential functions:
- Aligns the delivery of services with the goals and objectives of the client involving a significant amount of time working within an interdisciplinary team.
- Develops and monitors treatment plans, scheduling appointments, documentation of direct work with participants, identifying and addressing challenges existing within the programming, and presenting fresh ideas and services that would benefit the TNE community and client experience.
- Completes orientation with new clients and works with nurse to complete the state assessment upon admission, and as needed thereafter.
- Assists new clients with establishing medical care, including but not limited to?primary, vision and dental care. Works with clients to schedule follow up appointments or new referrals.
- Creates and updates weekly schedules for each resident. Schedules and attends treatment team meetings, and ensures treatment plans are reviewed and revised according to company policy. Monitors activity and group attendance.
- Works with nurse and other staff to ensure client has needed support with medications.
- Oversees resident pocket money and assists with budgeting. Assist clients with renewing or applying for vital documents and insurance when needed. Assists clients with problem solving, motivation, and engagement.
- Communicates directly with Program Manager, Support Staff, Clinicians, Skills Specialists, and external care providers as needed to organize the needs and goals of each client.
- Coordinates weekly meetings with individual clients, and weekly or as needed, family check-ins between clients and families/natural supports.
- Provides monthly updates summarizing the progress made by each resident throughout the month.
- Collaborates with others as needed to facilitate a monthly resident meeting focused on client ideas, input, and feedback to improve resident experience and foster engagement in treatment.
- Maintains therapeutic milieu through consistent adherence to community expectations.
- Shares on-call responsibilities with Program Manager.
- In collaboration with other residential staff, plan and organize recreational and social activities and trips with occasional overnight trips, weekend and evening activities and events. Supports with family visits and planning for visits off-campus.
- Leads discharge planning process with client, natural supports, and TNE treatment team.
- Other duties as assigned by supervisory personnel.

Working Conditions
Working conditions are normal for an office environment.? The noise level in the work environment is usually quiet to moderate.? Work will require weekend and/or evening work as needed.? Occasional conditions will vary as it will be necessary to be in various locations, homes of persons served or their families and funding agencies.? The employee must be able to travel between locations and occasionally attend seminars which may require an overnight stay.
Requirements/Qualifications
The person in this job will have the following experience and attributes:
- High School Diploma or equivalent
- Two years' experience in working with adults with serious and persistent mental illness
The following are preferred but not required:
- Associate's degree from an accredited college or equivalent experience
