Case Manager

Provides support and services to adults living with complex mental health diagnoses in a residential treatment setting. Coordinates the delivery of services in alignment with each resident's goals and objectives of the client. Functions as a collaborative member of an interdisciplinary treatment team. Facilitates communication between the individual client, the treatment team, external providers, and natural supports. Reports to the Program Manager of Averte's Main House (Trivium New England).

Essential Duties

This position performs, but is not limited to, the following essential functions:

  • Develops and monitors treatment plans, coordinates and schedules treatment team meetings, attends treatment team meetings, and supports the integration of treatment plan goals and objectives through ongoing monitoring and staff training.
  • Coordinates weekly meetings with individual residents. Establishes regular communication with families and natural supports. Maintains ongoing communication as needed. Provides regular updates regarding resident progress as appropriate.
  • Oversees resident schedules, including internal and external appointments (e.g. therapy, medical).
  • Supports orientation with new residents. Works with Admissions Coordinator to ensure new residents are provided the necessary information and support upon admission to Averte. Works with program nurse to complete the state assessment upon admission, and as needed thereafter. Works collaboratively with residents to establish a regular schedule.
  • Assists new residents with establishing medical care, including but not limited to primary care, vision, and dental care. Works with clients to schedule appointments.
  • Creates and updates weekly schedules for each resident. Schedules and attends treatment team meetings and ensures treatment plans are reviewed and revised according to company policy and licensing regulations. Monitors progress on identified goals and objectives.
  • Collaborates with team members to organize recreational and social activities for residents. Support resident engagement in programming in alignment with each resident’s treatment goals.
  • Documents direct work with clients, communication with families and care coordination with collaterals. Adheres to all standards relevant to client medical records, including program procedures, agency policies, licensing regulations and best practice standards.
  • Works with program nurse and other team members to ensure client has adequate support with medications.
  • Oversees resident money and assists with budgeting.
  • Assists clients with applying for/renewing vital documents and insurance as needed.
  • Assists clients with engagement, motivation, and problem-solving.
  • Communications directly with Program Manager, all members of the Averte treatment team, and external care providers as needed to organize the needs and services of each client.
  • Contributes effectively to a therapeutic milieu through consistent adherence to community expectations.
  • Supports the scheduling of family visits.
  • Leads discharge planning with residents, families and the Averte treatment team.
  • Other duties as assigned by supervisory personnel.
  • Attends and provides input of all required staff orientations, staff meetings. Completes all required training functions in a successful and timely manner
  • Complies and is aware of all HIPAA (Health Insurance Portability and Accountability Act) regulations that are required in this job

Required Knowledge, Skills, and Abilities (KSAs)

To perform this job successfully, the individual must have the following experience and attributes:

  • High School Diploma or equivalent
  • Minimum two years' experience in working in human services and/or mental health field
  • Demonstrates effective time management, prioritization, and coordinating satisfactory or above work outcomes and activities with others
  • Committed to clear communication, sharing ideas, and building and developing positive internal and external relationships
  • Demonstrates proficient computer literacy skills
  • Committed to self-improvement; ability to support and perform work through Company values
  • Valid Chauffeur’s license and a driving record that is acceptable to Company auto insurance company
  • Valid motor vehicle license and driving record acceptable to the Company auto insurance guidelines

Preferred Knowledge, Skills, and Abilities (KSAs)

  • Associate’s degree from an accredited college or equivalent experience.
  • Prior experience with case management in a related setting and/or field.

  • Working conditions are for a regular office environment. Noise level in the work environment is usually quiet to moderate.
  • Must communicate information and ideas so others will understand. Must be able to exchange accurate information in these situations.
  • Must observe details at close range.
  • Must be able to remain in a stationary position approximately 50% of the time. Occasionally moves about inside the office to access office equipment. Constantly operates a computer and other office equipment.
  • Must be able to travel between work sites occasionally to perform the essential functions of this job to support people served. May be required to travel and attend seminars/training or meetings occasionally which may require an overnight stay.
  • Provide moderate physical assistance to people served when needed.
  • Must be able to lift up to 20 pounds for various needs
