Position
To manage client's day to day milieu across all assigned settings, instruct, supervise, and assist clients with routine care and activities of daily living and with social skills as well, striving to improve each resident's quality of life.

Essential Duties
This position includes, but is not limited to, the following essential functions:

  • Treat people we serve with dignity and respect.
  • Provide day to day behavioral and environmental management of the milieu in an atmosphere of encouragement, acceptance, and unconditional regard for all clients.
  • Know and enact the mission of the organization in a positive manner.
  • Correctly interpret and apply rules, policies and procedures, regulations, and standards in a consistent, systematic manner.
  • Identify problem areas and help develop effective and efficient corrective action.
  • Participate in organizational meetings, special projects and other related duties for the organization.
  • Attend and participate in weekly staff meetings. All staff on shift are required to attend unless otherwise deployed by the Program Manager (PM). If you are not in attendance of any staff meeting, you are responsible to know all of the material that was discussed. This can be done during weekly/biweekly supervision meetings with the PM and by also reading the house notes in Best Notes. Staff are required to attend at least 5 house meetings a year.
  • Know and execute all emergency response procedures with appropriate personnel and associated phone numbers.
  • Review all Client Care Plans on admission and monthly thereafter and provide for their day-to-day implementation as appropriate.
  • Remain educated and prepared to help clients with self-administration of medication(s) accomplished in accordance with the medication policy.
  • Provide transportation /escort for any resident to and from any medical or non-medical appointments or other designated activities.
  • Monitor and maintain the physical environment ensuring all space and equipment is in good working order. Items or conditions requiring repair are to be communicated to the PM in a timely manner appropriate to its urgency.
  • Work with team and other organizational personnel to solve issues in a consistent manner.
  • Communicate clearly and effectively in both verbal and written fashion.
  • Always communicate in a positive and respectful manner to all clients, staff and visitors.
  • Utilize the electronic health record to effectively complete service-by- service documentation using a person-centered model and document all other supports, meetings and case coordination as appropriate.
  • Establish and maintain effective professional working relationships with all co-workers and clients.
  • Work positively with staff in a way that promotes the development of each other and contributes to strong staff morale.
  • Assist in the development of community-based experiences based upon persons served interests and staff's skill (such as nutrition, money management, social security benefits, etc.).
  • Advocate for persons served to be included within their communities, as they desire.
  • Facilitate integration and community inclusion.
  • Work as a team member with other staff to provide quality service to the people we serve.
  • Record direct persons served contact hours as instructed on all appropriate forms for efficient and cost-effective use of time.
  • Provide physical assistance to persons receiving services when needed (example 1-2 persons lift to transfer from wheelchair).
  • Document persons served interactions and support interventions in the form of contact notes/ ISP data, incident reports, T-log timely and as instructed.
  • Provide safe transportation and supports for persons to access their communities with alternate (safe) transportation methods.
  • Be responsive to and provide emergency intervention for all persons receiving residential supports.
  • Be responsible to the Program Manager and other designated staff and keep them aware of persons served and/or staff support needs.
  • Work cooperatively with other staff and with the person receiving services to provide supports needed and desired to achieve quality outcomes.
  • Work cooperatively with the persons receiving services and all stake holders (as desired) and their community options team to support the person's Person-Centered Plan.
  • Must have authorized release before sensitive information is given pertaining to persons receiving services, and staff.
  • Will be aware of and comply with all HIPAA regulations in coordination with the HIPAA Compliance Officer.
  • Will abide by all Trivium Life Services Corporate Compliance policies and Code of Ethics.
  • Communicate staff schedule needs/changes to the Residential Team Coordinator and other designated staff responsible for scheduling, in a timely manner and arrange for replacements with independence (getting approval if it will cause overtime) except for emergency situations.
  • Attend staff meetings and training opportunities as scheduled, providing input and feedback to the Program Manager concerning persons served support needs and service development.
  • Attend and successfully complete all necessary staff orientation and training functions.
  • Work a shift per an established schedule and be flexible to work outside the shift parameters based upon the needs of the business.
  • Other duties as assigned by supervisory personnel.

Physical Demands
These physical demands are representative of the physical requirements necessary for an employee to successfully perform the essential functions of this job.  Reasonable accommodation can be made to enable people with disabilities to perform the described essential functions of this job. 

While performing the responsibilities of this job, the employee is required to talk and hear.  The employee is often required to sit and use their hands and fingers, to handle or feel and to manipulate keys on a keyboard. 

The employee is often required to stand, walk, reach with arms and hands, climb or balance, and to stoop, kneel, crouch or crawl.  Vision abilities required by this job include close and distant vision. The employee must be able to travel between locations, and occasionally attend seminars which may require an overnight stay.

Working Conditions
Working conditions are normal for a home environment.  Based on the person served needs, the environment will change according to seasons while accessing the community.  The noise level in the work environment is usually quiet to moderate.  Work may require occasional weekend and/or evening work.

Requirements/Qualifications
The following experience and attributes are required in this job:

  • Must be at least 18 years of age.
  • High School Diploma or equivalent 
  • If the employee is driving a company vehicle, a valid and current driver's license and the ability to maintain a driving record acceptable by the company insurer guidelines are required.  The company will run a pre-hire and annual MVR to review the employee's driving record. 
  • If the employee is driving to another work location in a personal vehicle, the employee should have a valid and current driver's license.  Employees are not allowed to drive clients in personal vehicles.
