Independent Living Specialist

Essential Duties

This position performs, but is not limited to, the following essential functions:

  • Actively participate in fostering a therapeutic environment in a manner that facilitates progress toward individualized treatment goals.
  • Support resident engagement and social connection, both within Company programming and in the external community. Advocate for individuals served to actively participate and be included in their communities and facilitate meaningful community involvement aligned with their personal goals, preferences, and desires.
  • Support the design and implementation of individualized, community-based experiences that align with client interests and leverage staff expertise (e.g., vocational, educational, social skills, nutrition education, money management).
  • Regularly review all resident treatment plans and carry out identified interventions aligned with individual treatment goals. Collaborate with the Program Manager to ensure ILP treatment plans are current, comprehensive, and reflective of each client's evolving needs, goals, and preferences.
  • Actively participate in activity programming, community events, and in both group and individual endeavors that support residents' interests and treatment needs. This may include both on-site and community-based programming. Facilitate resident groups and lead activities designed to promote independent living skills, fostering personal growth, self-sufficiency, and community engagement.
  • Work collaboratively with the Program Manager to ensure each ILP client receives appropriate guidance and support through timely check-ins and coordination of medical, therapy, and other essential appointments.
  • Collaborate with relevant team members, including Program Managers and the Facilities team, to oversee all physical aspects of the homes, ensure comfort and safety, conduct regular apartment checks, and ensure proper maintenance and upkeep of the residence.
  • Support residents in activities of daily living (including but not limited to laundry, room checks, hygiene, nutrition, etc.) in accordance with individual treatment plans and best practices.
  • Establish positive therapeutic rapport with residents, maintaining therapeutic boundaries in accordance with policies, procedures, and assigned training content.
  • Supervise residents' self-administration of medication. Complete required training and review relevant policies, procedures and regulations pertaining to supervision of residents' self-administration of medication.
  • Provide transportation for residents to and from appointments or other designated activities as assigned by the Program Manager.
  • Provide emergency response and/or intervention for residents within the scope of training protocols, as outlined by policies and licensing regulations. This includes medical, psychiatric, or behavioral crises, and fire/emergency response protocols.
  • Communicate resident needs to Program Manager and other relevant team members in a timely manner in accordance with degree of urgency to promote safety and support for the individual and the milieu.
  • Utilize the electronic health record system to complete treatment documentation, as assigned by Program Manager and in accordance with policies, procedures, licensing regulations and best practice standards of person-centered care.
  • Utilize all electronic systems essential to assigned duties, including Microsoft 365, Datis, Relias, and NetSuite. Utilize Company email for internal communication. Check email during each shift to ensure important communications are received.
  • Attend and actively engage in regularly scheduled supervision. Communicate concerns and challenges to supervisors in a timely manner.
  • Exercise professionalism and conduct in line with the organization's mission.
  • Understand and uphold the Company's community expectations, policies and procedures, licensing regulations, and standards of care.
  • Monitor and maintain the physical environment ensuring all space and equipment is clean and in good working order. Items or conditions requiring attention are to be communicated to the Program Manager promptly. Uphold cleanliness standards to promote a therapeutic caregiving environment for residents.
  • Work as a collaborative member of an interdisciplinary team. Establish and maintain positive working rapport with other team members. Observe professional conduct and communication with other team members. Actively contribute to positive workplace culture and strong staff morale.
  • Consistently engage with clients, family members, external supports, vendors and visitors professionally and respectfully, in accordance with signed release of information and HIPAA regulations.
  • Work all regularly scheduled shifts and exercise a reasonable degree of flexibility to work in accordance with the organization's needs (e.g. unforeseen emergency-based staffing shortages). Communicate scheduling changes and/or needs to the Program Manager (or scheduling designee) in a timely and proactive manner and in accordance with the organization's policies and procedures. When feasible, find coverage for planned absences. Seek advance approval for overtime.
  • Attend and provide input on all required staff orientations, staff meetings. Completes all required training functions in a successful and timely manner. Attend all required staff meetings and training as scheduled, and complete all training assigned in Relias in a timely manner. If unable to attend required meetings or trainings, proactively communicate needs with Program Manager and make alternative arrangements to review all meeting/training content. Complete Relias trainings on or before due date in accordance with internal policies and licensing requirements.
  • Comply with all HIPAA (Health Insurance Portability and Accountability Act) regulations that are required in this job.
  • Abide by all Company policies. All job duties and responsibilities must be carried out in compliance with applicable legal and regulatory requirements and without discrimination.

Required Knowledge, Skills, and Abilities (KSAs)

To perform this job successfully, the individual must have the following education, experience and attributes: 

  • Must be 18 years of age.
  • High school diploma or equivalent required.
  • Minimum of 1 year work experience providing services to clients in a similar/related human services setting.
  • Demonstrates effective time management, prioritization, and coordinating satisfactory or above work outcomes and activities with others
  • Committed to clear communication, sharing ideas, and building and developing positive internal and external relationships
  • Demonstrates proficient computer literacy skills
  • Committed to self-improvement; ability to support and perform work through Company values
  • Valid Chauffeur's license and a driving record that is acceptable to Company auto insurance company

Preferred Knowledge, Skills, and Abilities (KSAs)

  • Two years of experience working in direct care or with adults with mental health needs preferred.
  • Additional education in a human service-related field preferred
  • Working knowledge of mental health diagnoses, needs and challenges through a person-centered approach.
  • Demonstrated ability to actively engage clients in identified individual treatment goals.

  • Frequently moves about inside and between/outside building(s) to assist residents. Frequently operates a computer and other office equipment such as a printer. Occasionally moves equipment or items up to 50 pounds. Occasionally stoops, kneels, crouches or crawls to assist residents. Occasionally ascends/descends a ladder or stairs to assist residents.
  • Working conditions are normal for a home environment. Based on the residents' served needs, the environment will change according to seasons while accessing the community. The noise level in the work environment is usually quiet to moderate.
  • Position may require weekend and/or evening work in order to support individuals served.
  • The employee must travel between locations and occasionally attend seminars, which may require an overnight stay.
